What does a visit look like?

After establishing your medical and dental health history profile we will complete a thorough examination. We check for things like bone loss, loose teeth, bite, oral cancer screening, TMJ and signs of periodontal disease.

Please assist us at the time of your initial visit to the office by providing the following information:

  • If you are being referred by your dentist, please bring your referral form and the X-Rays from your referring dental office.
  • A list of medications you are currently taking – See below to download our Medical History form and Registration Form.
  • Your dental insurance card or dental insurance information.

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.  Please notify us if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring premedication, severe diabetes, or hypertension.) We will complete an in depth medical and dental health history and a thorough examination to measure for bone loss, loose teeth, bite, oral cancer screening, TMJ and other signs of periodontal disease.
